Permit Application Requirement for Out of State Visitors

The following is required ONLY for visitors doing rotations in Texas as part of the visitor's out-of-state training program. A letter must be prepared and signed by the Texas licensed physician who will be supervising the visitor's Texas rotations here at UTHealth Medical School. This letter must be brought to Joan Flores in the GME Office. Joan will send the letter to the Texas Medical Board when they request it. Do not submit the letter directly to the Texas Medical Board. The letter to Texas Medical Board must certify the following:

  1. the facility at which the rotations are being completed
  2. the dates the rotations will be completed in Texas
  3. that the Texas on-site preceptor physician will supervise and be responsible for the applicant during the rotation in Texas
  4. signature of the supervising physician and department name
*Rotator permits are limited to the dates of the rotation in Texas. The permit will reflect the name and address of the out of state postgraduate training program, not the Texas program where the rotation is being completed. This letter must be in Joan's hands no later than 90 days before the rotation start date. Joan has prepared a template letter that meets the requirements. Please contact Joan if you have questions.
